Later, together with Oleh Dopilka and several other players, Milko entered the Kyiv sports boarding school. At one of the tournaments, Vadym was noticed by representatives of "Dynamo" Kyiv and in 2002 he joined the Dynamo system.
On July 25, 2002, Milko made his debut for "Dynamo-3", coming on the field in a match against Ternopil's "Niva" (3: 2).
Within a year, Vadim was transferred to the ranks of "Dynamo-2", as part of which he first appeared on the field on July 18, 2003 in a match against "Metalist" (7: 5). Confident play and authority among the partners led to the fact that Milko was elected captain of "Dynamo-2".
For the first time, rumors that Vadim Milko wants to try himself at a higher level and leave "Dynamo" began to spread in mid-2006. Since the young midfielder did not get to the main lineup, and the level of reservists, he had already outgrown at that time. However, the club's management persuaded him to wait some more time. On June 17, 2007, Vadim made his debut as a member of "Dynamo's" main team in the last round of the championship match against "Arsenal" Kyiv. Dynamo coaches put the second team on the game and Milko played on the field for 72 minutes, after which he was replaced by Denis Oliynyk.
Representatives of the Premier League, in particular the capital's "Arsenal" and Okhtyrka's "Naftovyk-Ukrnafta", were actively interested in the player, but Vadim still hoped to break into the "Dynamo" base. In the end, having played only one match in the first round of the 2007/2008 season, Milko went on loan to FC "Kharkiv". After spending a good period of time in Kharkiv, Vadym Milko was disappointed with this period of his career due to serious financial problems that arose with the club's management.
Having many offers from both domestic clubs and teams from near abroad (Russia, Azerbaijan), the footballer chose Luhansk "Zorya" to continue his performances.
On August 8, 2009, Vadim made his debut for Luhansk in a match against "Karpaty" (0: 4). And he scored the first goal for "Zorya" more than two years later, hitting "Shakhtar" Donetsk.
In the summer of 2012, Milko signed a 1 + 1 contract with Poltava's "Vorskla".
In the winter of 2013, Vadim signed a one-year contract with the Belarusian club "Bilshyna".
In 2014-2016 he played for the Belarusian team "Slutsk".After one match played for Rivne's "Veres" in the 2016/17 season, the following season Milko moved to Kovalivka, where he became a key player of the team and still enjoys authority both among the players and among the coaching staff.
Among the memorable matches for "Kolos", it is worth noting the game against "Shakhtar" Donetsk on March 8, 2020 at "Obolon Arena", in which "Kolos" lost to miners with a score of 3: 4, and Vadim scored a double in that match.
Vadym Milko played 38 matches for the youth national teams of Ukraine in different age categories, in which he scored 3 goals.
